Providing high quality, cost-effective care to more than 140,000 individuals in New York State each year, Visiting Nurse Service of New York (VNSNY) is the largest and one of the oldest not-for-profit home and community-based healthcare providers in the U.S. VNSNY meets the clinical and social needs of patients through a wide-range of programs, including: skilled in-home nursing and rehabilitation, hospice and palliative care, community mental health, children and family services, and community outreach. VNSNY CHOICE Medicare and Medicaid health plans further our mission of care and compassion for the chronically ill and the elderly.

VNSNY is actively engaged in a number of care innovations and value-based initiatives, including participating as a single awardee in the Bundled Payment for Care Improvement (BPCI) Model 3 for Congestive Heart Failure and Acute Myocardial Infarction; serving as the preferred home health agency for three BPCI Model 2 hospital participants; pioneering case rate episodes with risk-based payments based on quality outcomes in Medicare Advantage; and operating the largest Medicare-Medicaid Plan (MMP) in New York State’s CMS’ Financial Alignment Initiative.
