Primary Care Collaborative

Primary Care Collaborative, founded in 2006, is a not-for-profit, multi-stakeholder membership organization committed to advancing an effective, efficient health care system grounded in primary care and the patient-centered medical home (PCMH). Its members include a diverse mix of public and private organizations—payers, providers, nonprofits, and leading corporations—working together to transform health care.
